Research confirms the longer a person is away from work, the harder it is for them to return, In fact, after 6months, 50% of employees do not return to work and this increases to 80% after 12months.

By exploring early interventions, your organisation can support an employee
who is still in work but at risk of absence remain in work and avoid the absence, or
plan for a safe, timely and sustainable return to work.

How does the service work?

Successful case management is a collaborative process, focussed on developing and implementing a customized plan to support your and your employee stay-at or return-to work.

Working closely with HR Managers, Occupational Health or Line Managers, Connect4Work will give you the support you need to transform absenteeism into attendance.

The service is easily accessible with regular updates to ensure all parties involved are kept up to date. Each client will be given access to their own unique file which they can update in real time as they move through the following steps:

  • Referral from employer to Connect4Work
  • Meeting scheduled with employee
  • Stay-at or Return-to work Planning. Liaison with Allied Health Care Professional if required
  • Work plan drafted to include Reasonable Accommodations if/where appropriate
  • Assistance with Grant Applications where relevant
  • Implement plan, including contact points to ensure plan is progressing accordingly
